Mandatory Referral: Conditions of Employment (COE)

A supervisor or member of the Human Resources staff may mandate an employee to call the EAP as the last step of a progressive disciplinary process. This is called a mandatory referral or COE (Conditions of Employment). In this case, the employee, manager and a representative of Human Resources must sign the Conditions of Employment Agreement and fill out and sign the Conditions of Employment Information Form. The supervisor or Human Resources representative must then fax the forms to Charles Nechtem Associates, Inc. who will follow up to monitor the referred employee’s adherence to the agreed course of treatment. Charles Nechtem Associates, Inc. will provide progress reports to the designated supervisor or Human Resources manager on any employees referred for COE. Such reports will be limited to reporting whether or not the employee is cooperating with the treatment program.
